People's governments of all provinces, autonomous regions and municipalities directly under the Central Government, and ministries and commissions as well as institutions directly under the State Council,
Since the implementation of the new-round medical and health system reform, the establishment and refinement of China's universal medical insurance system has been accelerated, the service conditions of primary-level medical and health institutions have been significantly improved and the building of primary-level personnel teams with general practitioners as the focus has been continuously strengthened, with the situation concerning the long-term weakness of primary-level services changed gradually and the fairness and accessibility of basic medical and health services improved significantly. However, we must ensure that the building of a strong primary level is a long-term and arduous task, and the insufficient total amount, irrational structure and uneven distribution of quality medical resources, especially weakness in terms of the lack of primary-level personnel faced by China have become major constraints to guarantee the people's health and deepen the healthcare reform. The building of medical alliances conducted as an important step and institutional innovation for deepening the healthcare reform is conducive to adjusting and optimizing the structure and layout of medical resources, promoting the downward shift of the focus of medical and health work and the downward transfer of resources to the primary level and improving primary-level service capacity, as well as being conducive to connecting medical resources from higher to lower levels, raising the overall efficiency of the medical service system, and better implementing tiered diagnosis and treatment and meeting the needs of the masses for health. The following opinions are hereby put forward upon the approval of the State Council with a view to guiding all localities to promote the building and development of medical alliances.
I. General Requirements
1. Guiding ideology. We shall fully implement the spirit of the 18th CPC National Congress and the 3rd, 4th, 5th and 6th plenary sessions of the 18th CPC Central Committee as well as the National Conference on Health and Wellness, conscientiously implement the decision-making and deployments of the CPC Central Committee and the State Council, and coordinate the overall layout of the Five-sphere Integrated Plan and pursue balanced progress in the Four-pronged Comprehensive Strategy, firmly establish and implement the development concept of innovation, coordination, green, openness and sharing, adhere to the people-centered philosophy of development, and based on the actuality of the development of China's economic, social, and medical and health undertakings, with focus on implementing the functional orientation of medical institutions, improving the service capacity of the primary level and sorting out the two-way referral process, continuously improve the organizational management mode of and operation and incentive mechanisms for medical alliances, and gradually establish and improve a fair and effective mechanism for the division of labor and collaboration with definite objectives as well as clear powers and responsibilities among medical institutions of different levels and categories, promote the building of a tiered diagnosis and treatment system, and achieve the transformation from the disease-treating centered development mode to a health-centered one.
2. Basic principles
Carry out overall planning under the leadership of governments. We shall implement such governmental functions as planning, guidance, coordination, supervision and publicity, put emphasis on cities and counties, in light of the structural layout of regional medical resources and the health needs of the masses, in line with the requirements of business correlation, complementary advantages, bilateral selection and sustainable development, etc., and take into consideration the cooperative relationship formed previously to arrange the building of medical alliances by medical institutions in a coordinated manner.
Adhere to public welfare and innovate mechanisms. We shall adhere to governments' principal responsibility for running medical institutions, and effectively safeguard and guarantee the public welfare of basic medical and health undertakings; adhere to the linkage reform of medical treatment, medical insurance and medical products, innovate mechanisms, and gradually remove barriers and obstacles in terms of administrative divisions, financial investment, medical insurance payment and personnel management, etc., optimize the structural layout of resources, and in combination with the advancement of the reform of medical insurance payment methods, etc., gradually establish a sound division of labor and collaboration mechanism among medical institutions.
Transfer resources downward to lower levels and improve capabilities. We shall take advantage of the concentrated quality resources in Grade-Ⅲ public hospitals by means of technical assistance and personnel training, etc., give play to the role thereof in technically radiating towards and driving the primary level; encourage medical alliances to adopt the unified management mode, exert the advantages of intensification, promote the sharing of regional medical resources, give play to the leading and supporting role of science and technology, and improve the overall capacity and performance of the medical service system.
Provide convenience and benefit for the people and ensure the masses' benefits. We shall adhere to centering on the health of the people, gradually realize the medical quality homogenization management, strengthen the capacity of primary-level medical and health institutions as the "gatekeepers" for residents' health, promote the combination of prevention, treatment and management of chronic diseases, promote the linkage of the building of medical alliances with disease prevention and healthcare, make it convenient for the masses to seek medical advice nearby, reduce the burden of diseases, prevent poverty and from returning to poverty due to diseases, promote the development of the health industry and economic transformation and upgrade and enhance the masses' sense of satisfaction.
